Forms

Any changes to UltraTime may be requested via the forms provided on Payroll's website. Frequently used forms include:

• Non-Exempt Payroll Adjustment Form
  • Use this form to add hours or make adjustments to a previous pay period (missed punch)
• Request for New UltraTime Supervisor Form
  • Use this form to request supervisor access for someone to approve hours in UltraTime
• UltraTime Supervisor Change Form
  • Use this form to assign a new approving supervisor for an employee.
