Leave Checklist
The Leave Checklist, which specifies the forms and other materials needed for a leave application, should be included with each application. "Form L" and the "Form L Packet Enclosure Checklist" can be found on the Faculty Administration Support & Training site.
If a regular faculty member of Notre Dame will be going to an outside institution during their leave, typically as a visiting faculty member, they will need to have an Outbound Agreement completed once their leave has been approved.
